Position Summary

The H.E.A.L Team member will serve as the first point of contact when an individual eligible for the Life Outside of violence (LOV) program arrives at the Emergency Department of one of the two adult partnering Level 1 Trauma Centers (Barnes Jewish Hospital, SSM St. Louis University Hospital). The person in this position will provide outreach and information about the LOV program to potential participants while they are in the hospital or in the community.

Job Description

Primary Duties & Responsibilities:

Provide support to patients/families in the hospital at time of care for of injury.

  • Respond to in-coming patients and/or their families that have been violently injured or meet any other LOV criteria while they are in the hospital emergency department and/or inpatient trauma unit.
  • Check in with patients that meet LOV criteria and currently hospitalized for propose of enrollment.
  • Correspond with Outpatient H.E.A.L. Team worker to ensure proper communication.
  • Check in with hospital social worker to ensure all patients that are eligible for LOV programming have been identified and provided with crisis support and engagement for possible program enrollment.

Completion of required administrative tasks.

  • Track hours and mileage accurately for invoicing procedures. 
  • Provide regular reports on progress of work to supervisors.
  • Maintain list of LOV eligible participants currently in outreach.
  • Document all activity in a consistent, accurate and timely manner as required.

Maintain communication with LOV team to ensure quality service.

  • Participate in weekly team staff meetings.
  • Participate in daily check in meetings with supervisor.
  • Communicate through expected documentation processes (EPIC and RedCap).
  • Check email a minimum of once per shift.
  • Attend and participate in all required LOV staff trainings.
  • Attend and participate in professional development opportunities as identified by leadership.

Provide patients/families/communities with resources.

  • Make appropriate community referrals based on needs assessment.
  • Maintain relationships with other community members and organizations related to violence prevention activities and approaches.  
  • Provide follow-up crisis support and engagement as needed in efforts to support patients in the outreach phase.
  • Attend and help facilitate community events related to reducing community violence.

Salary Range

$17.34 - $25.40 / Hourly

The salary range reflects base salaries paid for positions in a given job grade across the University.
